VII. LEAVE OF ABSENCE:

In the event that a resident must request a leave of absence it is advised that the resident review the Resident Leave Policies contained in the Resident Handbook (found at www.medicine.nevada.edu/gme/UNSOM2007final.pdf) to determine his/her eligibility. Once the resident determines eligibility exists the leave must be initiated by the resident by completing the Initial Resident Leave Request Form and submitting the signed form to THEIR program's Residency Coordinator (to determine the appropriate Coordinator please review www.medicine.nevada.edu/gme/CoordinatorList.asp) with any required documentation (i.e. medical restriction signed by physician, adoption or other legal documents, etc.) as far in advance of the beginning leave date as possible.

In the event of a need to extend or revise an leave of absence which is already approved or has already begun the resident must complete a Resident Leave Request Revision Form and submit the signed form to THEIR program's Residency Coordinator with any required documentation immediately upon knowledge of such need.
