Step I: Know your Residency Coordinator and his/her needs
(click here for list by program)

Your program coordinator is your ADMINISTRATIVE CONTACT to your residency program. Your program coordinator will have program-specific requests for you. At this time, please send a copy of your final diploma to your contact. He/she may require additional items from you.

Step II: Licensing in Nevada

In order to practice medicine in Nevada, you will need to have a medical license. Residents need to obtain training or restricted licenses from either the Medical or Osteopathic Board and fellows need to obtain an unrestricted from the appropriate board. In most cases the applications have been included in the initial packets sent out by the coordinators. These applications are due April 1, 2008.

Step III: Hospital Credentialing Forms

In order to help you complete the necessary forms to begin your training at various hospitals, we have tried to put the necessary forms in one location for your convenience. Some of the hospitals have already sent out information, and will not be part of this process.

Residency Program Hospital Grid

Instructions: Please click on the link above and locate your program. The hospitals at which you will be rotating are designated by the letter X. If there is a blue X, a link has been established to information which you need to read. In many cases there are tests and other forms that need completion. Often there is a checklist that also needs to be completed. Please follow the instructions for the individual sections from each hospital in your program.

Pay close attention to where things need to be sent and the deadlines for completing and sending in forms.

Step IV: Important Forms including GME Forms, Human Resource Forms and Benefit Forms

This step provides you with important forms that are needed for orientation and securing employment at the University of Nevada School of Medicine. Further, these forms will facilitate your signing up for your various benefits.
