Residents should obtain forms from the Program Director’s office. Faculty may complete the form below and and it to Karen Waite, who will proof-read the form, forward it to the appropriate hospital(s) and pay the appropriate fees. For information regarding VA staff privileges, you may e-mail Alleen Porter by clicking on her name below.
