This one-year ACGME Approved Family Medicine Obstetrics Fellowship is affiliated with the University of Nevada Department of Family and Community Medicine, the Renown Regional Medical Center, the Renown Pregnancy Center and the Medical Group of Northern Nevada.

General Information
The University of Nevada School of Medicine Department of Family Medicine fellowship in obstetrics provides Board-certified family physicians an extended period of supervised training on a busy obstetrics service at Renown Medical Center in Reno, Nevada.
Fellows complete this intense twelve-month experience with depth of knowledge and experience in maternal/child care not possible in the traditional 36 month Family Medicine residency. Fellows advance procedural skills and competence throughout yearlong training. Anticipated procedural competence to be gained includes: vacuum assisted deliveries, uterine dilatation, suction and curettage and cesarean sections.

Goals and Objectives
MISSION
Provision of advanced training in comprehensive maternal/child care to Board certified family physicians.
GOALS
To develop advanced competency in prenatal care complicated by diabetes, hypertension, twin gestation; thyroid, cardiac and renal disease as well as mental health disorders.
To acquire advanced skills in the management of patients presenting with preterm labor, pre-eclampsia, diabetes, and placental disorders.
To attain competency in operative obstetrics, including cesarean sections, instrumented deliveries (vacuum assisted), tubal ligation, and amniocentesis as well as post-operative care of such patients.
To obtain experience in forceps deliveries.
To develop competency in the performance of antenatal tests such as obstetrical ultrasound, non-stress tests and biophysical profiles.
To obtain experience in counseling families regarding genetic and environmental risks to reproduction.
To develop advanced skills in the resuscitation of distressed newborns and to provide ongoing care to the hospitalized newborn.
To maintain Family Medicine clinical skills.
To develop competency in teaching comprehensive reproductive care to family medicine residents.
OBJECTIVES
To provide initial consultation and ongoing prenatal care to patients in the clinical setting precepted by UNSOM faculty obstetricians and/or perinatologists.
To admit and manage patients to the antepartum and obstetrical services under the direction of faculty attendings.
To perform non-stress and stress tests, ultrasounds and biophysical profiles and amniocentesis for appropriate indications to patients on the antepartum service under the supervision of faculty attendings.
To perform initial and ongoing evaluation of obstetrical patients on labor and delivery as well as appropriate labor management through interpretation of clinical assessment and maternal/fetal monitoring.
To provide normal spontaneous vaginal as well as instrumented vaginal deliveries (vacuum and forceps) to obstetrical patients.
To perform cesarean sections as primary surgeon under the direction and supervision of faculty attendings.
To improve surgical skills by assisting in gynecologic procedures including abdominal and vaginal hysterectomies.
To perform postpartum tubal ligation on patients desiring sterilization.
To co-counsel patients referred to genetics counselors in the clinical setting.
To perform newborn resuscitations including intubations and umbilical vein catheterization under supervision of the neonatologist.
To gain experience in the management of newborns in the newborn nursery as well as the neonatal intensive care unit under the supervision of the neonatologist.
To maintain family medicine skills by precepting continuity clinic one half day per week in the Department of Family Medicine.
To teach family medicine residents and medical students the fundamentals of obstetrics including triage evaluation, cervical examination, labor management, pitocin augmentation as well as IUPC, FSE, amnioinfusion and vaginal delivery.

Requirements
Successful applicants must qualify for Board Certification in Family Medicine and licensure in the state of Nevada. Applicants must submit the application form, medical school transcripts, copies of USMLE scores, and the names of three individuals qualified to write letters of recommendation on their behalf to be considered for the fellowship. A letter of recommendation from the applicants residency Program Director is also required. All application materials must be received no later than December 31, 2007.

Obstetrical Experiences
The UNR Family Medicine Residency program is offering a one-year, full-time OB Fellowship that provides training in:
-Low-risk obstetrics
-High-risk obstetrics
-Family-centered prenatal care
-Surgical experience as both first assist and primary surgeon for Cesarean sections
-Obstetric ultrasound
-Colposcopy
-LEEP procedures
The graduating Fellow will have obtained significant skills in the following areas. All procedural skills are approximated in the quantity of experience which will be obtained.
Cesarean Sections 100-200
Vaginal Deliveries 200-400
Surgical Deliveries (forceps or vacuum) 25-50
Manual removal of placenta 20-50
Post Partum D&C 30-50

Management of
Common perinatal conditions: antepartum hemorrhage, premature labor, postmaturity, IUGR, fetal demise, fetal distress, malpresentation, abnormal labor progress, postpartum hemorrhage, retained placenta, complicated laceration repairs
Shoulder Dystocia
High Risk Pregnancy: preeclampsia, preterm labor, gestational diabetes, etc
Medical Complications of Pregnancy: hypertensive disorders, infections in pregnancy, chorioamionitis
Teaching: The OB Fellow is responsible for teaching the PGY-1 on the Obstetric service and the students and residents on the Family Medicine Service and at the Family Medicine Center. Didactic teaching with presentations, textbooks and articles occurs weekly. A Journal Club and lectures are scheduled monthly.
Precepting: The OB Fellow will precept residents for 1 half-day per week at the Family Medicine Center
Call Responsibilities: Fellowship responsibilities are governed by the 80-hour workweek mandated by the ACGME. Night call averages 2 nights per week. Call duty responsibilities include patient care on labor and delivery, antepartum, postpartum, and the emergency department. Fellows are granted one day off in seven, which is free of professional responsibilities.

OB Fellowship Application Information
University of Nevada, Reno, Family Medicine Residency Training graduate medical education program is accredited by the Accreditation Council for Graduate Medical Education. The fellowship is not accredited by the ACGME as such accreditation is not available for obstetrical fellowships.
Requirements
Applicants must obtain an unrestricted license in Nevada by the beginning of the fellowship and have completed a 3-year ACGME-accredited Family Medicine Residency and obtain an unrestricted Nevada medical license and DEA by the beginning of the fellowship. US-licensed physicians who graduated from Family Medicine programs in the UK and Canada are also eligible.
University of Nevada does not sponsor Visas.
